Dear Friends,

As we launch our inaugural quarterly newsletter, I want to take a moment to celebrate what we have accomplished together and share our excitement for the year ahead.

Because of your support, 2025 was a year of powerful impact. We provided over $10 million in services to nearly 5,000 patients, including connecting our patients to more than $8 million worth of life-saving medications. We also continued to decrease emergency room usage among our patients, reaching our lowest number of ER visits since 2021, an important sign that access to consistent, preventive care is truly making a difference.

These milestones represent lives changed, families supported, and communities strengthened through access to quality, affordable health care. Every achievement has been made possible by our supporters, donors, volunteers, partners, board, and staff. Your commitment has brought us to this moment, and it is something worth celebrating.

As we step into a new year, we do so with hope, momentum, and a renewed commitment to growing our mission of changing lives, families, and communities through compassionate care. Thank you for being an essential part of the Partnership Health Center community.

Partnership Health Center (PHC) has received a $20,000 grant from the Georgia Charitable Care Network, provided by Kaiser Permanente, to expand its Remote Patient Monitoring (RPM) Program. RPM supports patients managing chronic conditions like diabetes and high blood pressure, particularly those facing barriers such as transportation challenges, financial strain, or the need for frequent follow-up.

Through regular telehealth check-ins, RPM keeps patients connected to their care team for earlier intervention and improved disease control. Support from a previous grant expanded access to medications, diabetes education, and nutritional counseling, resulting in meaningful outcomes, 56% of RPM patients saw significant A1C reductions, averaging a 10% decrease. The new funding will enhance patient education and strengthen data collection as the program enters its next phase.

PHC’s Telehealth and RPM programs are led by Director of Telehealth Jacqueline Pospisal, PA-C, and Megan Hoover, NP, whose personalized, patient-centered approach has driven the program’s success. With Jacqueline already certified as a Diabetes Educator and Megan pursuing certification, PHC continues to strengthen its chronic disease care through innovative telehealth services.

This past Wednesday, we had the honor of hosting a Board & Friends Clinic Tour, welcoming several PHC board members, current donors, and representatives from organizations across our community. It was a fantastic turnout, and we were thrilled to open our doors and offer an inside look at the work happening in our clinic every day.

Guests had the opportunity to see our mission in action, connect with staff, and learn more about how PHC continues to serve patients and partners throughout the region. Sharing these moments, and the impact behind them, made the afternoon especially meaningful.
